We will ask for you personal details and medical history to open a medical file and store it on our electronic medical system.

Your Confidentiality is taken very seriously at our office. You can be assured that our staff and doctors will not release any medical information without your consent. There is a confidentiality form in your packet that we need you to fill out.

If you would like to remove your details from our system you can request in writting and we will remove any of your personal or medical records.
